Hospital Form CMS-2552-10, Worksheet S-3 Part II, line 11: column 4 contract-labor dollars divided by column 5 paid hours. PayBeacon filters implausible records, winsorizes tails, and takes state medians. This line blends direct-patient-care roles; it is not a nurse-only job rate.

Published legal ceilings
Oregon, Minnesota, and Massachusetts publish temporary-staffing rate ceilings. A ceiling is not a typical wage. PayBeacon applies a cap only to RN modeled facility cost where the pinned rule is a suitable hospital RN ceiling; other values remain editorial context.
Release controls
Candidate sources must pass schema, coverage, sample, national-change, and state-continuity checks. An anomalous release is quarantined before it can replace last-good derived data. Current machine-readable profile: source-profile.json.
